AFTER HOURS ARE: 5PM-9AM WEEKDAYS 24 HOUR WEEKENDS/STATUTORY HOLIDAYS.
DO NOT CALL THE LONGTERM CARE ON-CALL PHYSICIAN OR NURSE PRACTITIONER DIRECTLY.
To speak with a physician or nurse practitioner after-hours about a long-term care patient, please check whether their most responsible physician (MRP) or nurse practitioner is part of the Long-term Care Initiative - see physician and nurse practitioner list, above. If so, follow these steps:
1. Prepare to call - Gather patient information (chart + medication) and start SBAR. Include the following information: a. Relevant past medical history [ex: previous heart attack, stroke, psychiatric history, why patient is in LTC] b. Relevant events [ex: labs, antibiotics] c. MOST level of intervention d. Allergies e. Last creatinine level/GFR f. Indicate if any blood thinners g. Vitals
2. Call Personal Touch On-Call Service at (604) 528 – 4658. Ask to be patched to the North Shore LTCI on-call physician or nurse practitioner. When speaking with on-call physician or nurse practitioner, include: a. Your name + where you are calling from b. Patient name + date of birth + PHN c. Patient MRP d. Major reason that patient is in LTC (ex: dementia, Parkinson’s, stroke) e. MOST level of intervention f. Provide details from the SBAR and any requested info from chart
3. After the call - Fax completed SBAR (including orders received and care provided) to on-call physician or nurse practitioner and to patient’s MRP. Include: a. Patient name + PHN + DOB + facility phone # (direct ward line)
*Note: LTCI physician and nurse practitioner fax numbers are sent to your facility. If needed, Personal Touch can provide you with the physician’s or nurse practitioner’s fax number.
